Step 1:

Our Scheduling Coordinators, Tamika and Sandy, will assist in reserving your visit with us.  We offer Virtual Visits as well!

Step 2:

Provide Dental and/or Medical Insurance (if necessary)

Please keep in mind that your insurance policy is a contract between you, your employer, and the insurance company. We must emphasize that as dental care providers, our relationship is with you, not with your insurance company.


  • Patient's driver’s license
  • Patient’s DOB and SSN
  • Dental insurance information (primary and secondary)
  • Medical insurance information (primary and secondary)
  • Policy holder’s DOB and SSN


 While the submission of insurance claims is a courtesy that we extend to our patients, all charges are your responsibility at the time of service.  It is the responsibility of the patient to provide us with the following information for a claim to be processed:

*AOSC is NOT a party within your contract*


Primary insurance - Health insurance plan that covers a person as an employee, subscriber, or member which is billed first when you receive health care.  For example, health insurance you receive through your employer is typically your primary insurance.


Secondary insurance - Health insurance plan that covers you in addition to your primary insurance plan.  Secondary insurance may help cover additional health care costs when your primary insurance plan is exhausted.  For example, if you already have insurance through your employer and choose to enroll with your spouse’s health insurance plan (if allowed), that coverage would become your secondary insurance.

How Your Insurance Claim is Processed . . .

Step 1: Bring pertinent insurance information to the consultation appointment (driver's license, medical and/or dental health cards) to enable us to submit a claim on your behalf. 


Step 2:  All insurance claims (MEDICAL and/or DENTAL) will be filed by our Insurance Team after treatment is rendered.


Step 3: If you are utilizing your DENTAL insurance and it's a medically-related treatment, it is mandatory for us to obtain a copy of your MEDICAL EXPLANATION OF BENEFITS (EOB) which will mailed to you by the insurance company.


**Our office does NOT receive a copy of this document by your insurance company**

**The insurance company will STOP the reimbursement process if this document is not submitted**


Step 4a: Mail the copy of your MEDICAL EXPLANATION OF BENEFITS (EOB) to our office address

Step 5: Once AOSC obtains a copy of your MEDICAL EXPLANATION OF BENEFITS (EOB), our office will submit this document to your DENTAL insurance electronically for reimbursement.
